Abstract: 

Introduction: The use of abdominal zippers for temporary abdominal closure was devised to facilitate repeated explorations, allowing multiple cleansing of the peritoneal cavity and the detection and management of septic complications. In our study, an open abdomen using a sterile zip in a patient with gangrenous bowels and reanastomosis derived infection is reported.Case report: In this study a patient with a history of appendectomy one year before referral (had referred to the physician with a one month complaint of abdominal pain) is presented. The patient underwent laparatomy twice. Most of his small intestine was resected due to the gangrene and primary anastomosis was performed. Finally after three days, the patient underwent multiple cleansing of the abdominal cavity following purulent discharge from the incision, acute respiratory distress syndrome and sepsis with diagnosis of discharge from the site of anastomosis. Zipper locked method was used for temporary closure of abdomen wall in order to facilitate multiple access of abdominal cavity. Finally, abdominal wall was closed constantly.Conclusion: Temporary closure of the abdomen by means of devices such as zip gives the opportunity of rapid, easy and safe assessment, management and treatment of these patients to the surgeons.

Abstract: 

The relative growth of the fiddler crab Uca sindensis in Pohl Port was studied. Ten 0.5 m2 quadrates were randomly sampled monthly during low tide periods from October 2009 to September 2010. A total of 840 crabs, 449 (53.45%) males and 391 (46.5%) females, were sampled. The Carapace width of males ranged from 5.5 to13.5 mm (mean±SD) (8.92±2.04), and of females from 5.0 to 11.5 mm (mean±SD) (7.95±1.34). Carapace height (CH) and Carapace length (CL), height of the major cheliped (HMC) of males, abdomen width of females (AW), major cheliped length (LMC), dactiluse (D), feeding cheliped (CHF) and merouse (M) were employed as dependent variables and carapace width (CW) as independent variable. The relationship between CW×CH was positive in males and females. The equation (Log CH=Log-4.881+4.681LogCW) for males (P<0.05) and Log CH=Log-1.882+3.299LogCW for females (P<0.05). The females showed allometric positive growth for CW×AW relation to male (Log AW=Log0.784+1.217Log CW) (P<0.05). Also four male crabs were found with two major cheliped in this study. Remarkable ontogenetic changes were observed in the allometric growth of the male major cheliped and the female abdomen, indicating that these structures are closely connected to the timing of sexual maturity. The allometric growth of U. sindensis in Pohl Port mangrove differed from other Uca populations so far studied, indicating that growth could have been influenced by environment variables such as food availability, population density, distribution of vegetation, sex ratio, soil temperature, organic matter, different of surface and sediment.

Abstract: 

Echinococcosis is a parasitic disease that is produced by Echinococcus granulosus and still represents an important medical problem in many countries. Echinococcosis in extrahepatic sites is usually asymptomatic unless the cyst causes symptoms due to pressure, as in our case, or ruptures to the peritoneal cavity. Echinococcal cysts at unusual locations many times pose diagnostic dilemmas, and the diagnosis sometimes is made intraoperatively. Primary ovarian and paraovarian cyst hydatid disease is a very rare entity. In our case, a 25-year-old female gravida 1, para 1, was admitted to the emergency department of the Alzahra Hospital of Tabriz due to diffuse abdominal pain, nausea, abdominal distension and urinary frequency of ten-day duration and exacerbation of pain from a few hours before admission. Clinical examination revealed tachycardia of 105 per minute, normal body temperature and normal blood pressure. Abdominopelvic sonography revealed a mass with regular thick contour located in the left ovary suggestive of huge cystadenoma or carcinoma of the ovary. Emergent laparotomy was performed for suspected adnexal torsion. Intra operative findings were a large paraovarian mass with thick wall without any torsion. Mass was removed completely and sent for frozen section study. Pathological examination showed the diagnosis of hydatid cyst. The postoperative period was uneventful and the patient was discharged 3 days later, with the advice to receive Albendazole. The patient referred to a general surgeon for management of the hepatic cysts.

Abstract: 

The term acute abdomen became popular in the beginning of the last century, and it meant that the surgeon is faced with an abdominal event or surgical abdomen that needs emergent surgery, and morbidity and mortality dramatically increased if surgery postponed. During the years better paraclinical and laboratory modalities became routine, and the surgeons were able to diagnose the event exactly and act accordingly in most usual cases; so the old saying “ open and see, rather than wait and see” became less reliable. Since acute abdominal conditions has more or less the same clinical and Paraclinical Signs and Sympatoms; the present article is meant mostly to deal with special cases in patients with comorbidities, because due to their compromised immune response, the clinical and laboratory findings has not changed pathological. These special conditions includes children, elderlies, morbid obese, diabetics, renal failure, postoperative cardiac patients and comorbidities as well as heavy smokers, alcohol and substance abusers; and last but not least patient with severe immune deficiency like HIV and the like; which needs special attention and frequent clinical and paraclinical evaluation for proper decision making.

Abstract: 

A woman presenting with severe right lower quadrant (RLQ) abdominal pain is presented here. She was evaluated for all usual cause of such pain. Laboratory tests and abdominal and pelvic sonography was normal. MRI revealed L1-L2 disc herniation.

Abstract: 

Esthetics is one of the prosthetic treatment goals. Correct selection of anterior teeth, regarding size and appearance, results in a successful treatment. The goal of the present research was to investigate the relationship between interzygomatic width and maxillary anterior teeth width in order to find a better determinant for artificial anterior teeth selection. In this study, the interzygomatic width and maxillary central incisors width and also the width of six maxillary anterior- teeth, in 71 subjects (29females and 42 males) were measured. The results showed a significant difference in maxillary anterior teeth width, comparing men and women, (P<0.05). These measurement in people under study, are less than Europeans. The interzygomatic width was also more in men than women (P<0.05), which is, less than of European race. The ratio between interzygomatic and central maxillary incisor width was 15:1 which is less than 16:1 that is used in clinic. Thus, it is recommended to pay more attention to anterior teeth selection, especially in young patients.
